Before you ask

What does it cost the patient?

Nothing out of pocket. Supports are funded through the participant's NDIS plan. If funding isn't in place yet, we'll review the situation with the family at no cost and help them get the right supports in their plan.

What if they're not on the NDIS yet?

Refer them anyway. We'll talk with the family about access options and interim arrangements, and we'll tell you straight away what is and isn't possible β€” within 24 hours.

Which areas do you cover?

We support participants Australia-wide, with active participants across six states and territories. Send the suburb and we'll confirm coverage on the same call.

Who actually responds to the referral?

A named Well Team coordinator owns your referral end to end β€” same person, direct contact, written updates until supports commence. No call centre, no ticket numbers.
